To receive this weapon, you must be in combat against an Orc. The orc must be either flat-footed, dead or grappled for this weapon to be achieved. While keeping the Orc flat-footed, you can call your shot if you roll over its AC while doing all you must to break what is required to call a shot to the spine and tear out its spine. In grapple, you can call a shot on its head if you possess the appropriate feat to attack while in a grappled stance. If the Orc is dead, then you must take a full round action to sever the skull and spine from the body, making any specific check rolls your DM make seem worthy to do this task.
